Missed a weekly GLP-1 dose? What the labels say

Each weekly GLP-1 medication has its own missed-dose window, and they are not the same. Here is what the official US prescribing information and Canadian product monographs say, side by side. Your prescriber's or pharmacist's instructions come first.

Missed-dose windows at a glance

MedicationTake it late ifSkip it ifChanging your weekly day
Ozempic (semaglutide)Within 5 days after the missed doseMore than 5 days have passedAt least 2 days (48 h) apart
Wegovy injection (semaglutide)Next dose is more than 2 days (48 h) awayNext dose is less than 2 days awayNot specified; ask your pharmacist

Zepbound (tirzepatide)Within 4 days (96 h) after the missed doseMore than 4 days have passedAt least 3 days (72 h) apart

In every case the labels say to go back to your regular once-weekly schedule afterward. None of them say to take two doses to catch up.

Ozempic

The US prescribing information says to take a missed dose "as soon as possible within 5 days after the missed dose." After 5 days, skip it and inject on your usual day. Health Canada's Ozempic product monograph uses the same 5-day rule, and its patient section adds: "Do not take an extra dose or increase the dose to make up for a missed dose."

Wegovy injection

Wegovy's rule counts forward to the next dose instead of back to the missed one. Per the US label, if the next scheduled dose is more than 2 days (48 hours) away, take the missed one as soon as possible. If it is less than 2 days away, skip it and resume on your regular day.

The label also says that if 2 or more doses in a row are missed, the dose is restarted at a lower level to reduce the risk of stomach side effects. That is a prescriber decision, so call them before restarting. Wegovy tablets have a different rule (skip the missed tablet and take the next one the following day).

Mounjaro and Zepbound

Both tirzepatide labels, and the Canadian Mounjaro product monograph, give a 4-day (96-hour) window. Within 4 days, take the missed dose as soon as possible. After that, skip it and take the next dose on your scheduled day. You can move your weekly day if the two doses are at least 3 days (72 hours) apart.

Making missed doses less likely

Keeping a record also makes injection site rotation and tracking side effects easier to discuss at appointments.

Frequently asked questions

I missed my Ozempic dose. How long do I have?

The Ozempic label (US) and product monograph (Canada) both say to take the missed dose as soon as possible within 5 days after it was due. If more than 5 days have passed, skip it and take the next dose on your regular day.

What if I missed my Wegovy injection?

The US Wegovy label says to take it as soon as possible if your next scheduled dose is more than 2 days (48 hours) away. If it is less than 2 days away, skip it and go back to your usual day. If you miss 2 or more doses in a row, the label says dosing is restarted at a lower dose, so contact your prescriber before your next injection.

What about Mounjaro or Zepbound?

The tirzepatide labels (Mounjaro and Zepbound in the US, and the Canadian Mounjaro monograph) give a 4-day (96-hour) window. After 4 days, skip the missed dose and take the next one on your regular day.

Should I take two doses to catch up?

No. None of these labels tell patients to double up. The Canadian Ozempic patient information says not to take an extra dose or increase the dose to make up for a missed one.

Can I move my injection day?

The Ozempic label allows changing the weekly day as long as doses are at least 2 days (48 hours) apart. The Mounjaro and Zepbound labels require at least 3 days (72 hours). Tell your pharmacist or prescriber when you change it.
