How many doses are in a GLP-1 pen?

Some GLP-1 pens hold one dose, some hold four, and a few Canadian Ozempic pens hold eight. Knowing which one you have tells you when to refill and when to throw a pen away. Here is what the US prescribing information and the Canadian product monographs say for Ozempic, Wegovy, Mounjaro and Zepbound injections. Your prescriber's and pharmacist's instructions come first.

Ozempic pen: 4 doses (US 1 mg and 2 mg pens), 4 or 6 doses (the 2 mg starter pen), and in Canada also 8-dose pens. Discard 56 days (US) or 8 weeks (Canada) after first use.

How long a pen lasts

For once-weekly injections, a pen's doses translate directly into weeks, but the in-use limit can end a pen first:

Two points from the labels matter for planning. First, the 56-day and 8-week clocks start at first use, so a delay between doses eats into them; the US Ozempic instructions say to write the disposal date on your calendar. Second, the tirzepatide KwikPen and multi-dose vial have a short 30-day window, so a late dose or a pause can mean throwing away a pen with doses left. If your schedule changes, ask your pharmacist how to handle the pen you have.

Why there is medicine left in the pen

Finishing your doses and still seeing liquid in the window is expected. The Canadian Mounjaro patient information says that after the 4th dose "you will see leftover medicine in the KwikPen. This is normal. The leftover medicine ensures that the KwikPen works correctly." The KwikPen will not let you dial a full dose after the 4th one. The US Zepbound and Mounjaro instructions say "Do not inject the leftover medicine" and not to transfer it into a syringe, because trying to use it "could result in an incomplete dose."

For Ozempic, the dose counter is the check. If it stops before your full dose is shown, the US and Canadian instructions say there is not enough left for a full dose: do not use it, and use a new pen. Do not count the pen clicks. The Canadian Ozempic and Wegovy monographs also state that patients should not administer the full volume of the pen at any time.

First use of each new multi-dose pen

Ozempic and Wegovy FlexTouch pens need a flow check before the first injection from each new pen only: the US Ozempic instructions say to dial to the flow check symbol, hold the pen needle up and press until a drop appears, repeating up to 6 times and changing the needle if there's still no drop. KwikPens have their own first-use step in their instructions; follow the leaflet in your carton.

Never share a pen

Every multi-dose pen in this guide is labelled for a single patient. The US Ozempic label says a pen "must never be shared between patients, even if the needle is changed," and the Mounjaro and Zepbound instructions say the same, because shared pens can spread serious infections.

Track pens, not just doses

Log the date you start each pen, so you know its 30-day or 56-day discard date.
Count injections per pen, so a 4-dose, 6-dose or 8-dose pen never surprises you.
Note the refill date before the last dose, not after it.

On a once-weekly schedule, a 4-dose pen covers 4 weeks, the US starter pen used as 4 × 0.25 mg plus 2 × 0.5 mg covers 6 weeks, and a Canadian 8-dose pen covers 8 weeks. Whatever is left, the labels say to throw the pen away 56 days (US) or 8 weeks (Canada) after first use.

How many doses are in a Mounjaro or Zepbound KwikPen?

Four. Every KwikPen strength, from 2.5 mg to 15 mg, holds 2.4 mL and delivers four 0.6 mL weekly doses. The labels say to throw it away after the 4th dose or 30 days after first use, even if medicine is left in it.

Why is there medicine left in my KwikPen after 4 doses?

The Canadian Mounjaro patient information says leftover medicine after the 4th dose is normal and ensures the KwikPen works correctly. The pen will not let you dial a full dose after the 4th one. The US and Canadian labels say not to inject the leftover medicine or transfer it into a syringe.

Can I get an extra dose out of my Ozempic pen?

How many needles come with an Ozempic pen?

In the US, the 0.25/0.5 mg pen carton includes 6 NovoFine Plus needles and the 1 mg and 2 mg pen cartons include 4. Canadian cartons hold 6 or 8 needles for the 0.25/0.5 mg pen, 4 or 8 for the 1 mg pen and 4 for the 2 mg pen. Use a new needle for each injection.
